Application of the PSSR Element of the PSM standard

Process Safety Management (PSM) Standard (CFR 1910.119) is a regulatory standard established by the Occupational Safety and Health Administration (OSHA) to manage hazards associated with highly hazardous chemicals. The main goal of PSM is to prevent accidents such as chemical spills, fires, and explosions by having processes that are designed, operated, and maintained safely. Many companies create a generic checklist and have it pencil-whipped, calling that their PSSR. This is dangerous because most industrial accidents occur during startup and shutdown. The initial startup is particularly hazardous because the equipment, and often the chemicals, are new to the operations and maintenance personnel.
